STUDENT PRACTICE SERVICE AGREEMENT DISCLOSURE

Services cannot be performed if a client has or reports:

• Pregnancy or breastfeeding (body contouring services only)

• Open wounds, infections, or active skin conditions

• Pacemakers or metal implants (RF services)

• Recent surgery (within the past 6 months)

• Uncontrolled diabetes

• Active cold sores or herpes outbreaks

Clients are responsible for full and honest disclosure during intake.

Student Status Disclosure

All cosmetic services are performed by a student practitioner currently enrolled in esthetics training.

Services are provided for educational and skill-development purposes and are not licensed services.

Compensation Disclosure

Fees paid reflect student practice rates and are applied toward:

   •    Time and hands-on training

   •    Use of professional supplies

   •    Educational experience required for licensure

Pricing does not imply licensed or medical-grade services and does not guarantee specific results.

Travel fees are:

• Non-refundable

• Due in advance

• Required before confirmation

Makeup by Asia, LLC is not responsible for delays caused by traffic, weather, or travel conditions.

Minor Consent PolicyClients under the age of 18 require written consent from a parent or legal guardian.

Certain services may be limited based on age, skin condition, or safety considerations.

By proceeding with booking, purchasing, or enrollment, clients confirm they have reviewed, understand, and agree to all terms outlined on this page.

By booking or receiving services, I acknowledge and agree that:

• I understand services are performed by a student practitioner

• Results may vary due to the educational nature of services

• I am participating voluntarily as a practice client/model

• I release the student practitioner from liability related to skill development

• I will not pursue legal, licensing, or regulatory claims based on student status

• I understand pricing will increase upon licensure

Makeup by Asia, LLC reserves the right to respectfully refuse or discontinue any service if a client:

• Is disrespectful, aggressive, or inappropriate

• Refuses to follow safety or intake instructions

• Attempts to pressure pricing or expectations

• Expects licensed, medical, or guaranteed outcomes

• Compromises the learning or service environment

This policy exists to protect safety, professionalism, and educational integrity.

Student Practitioner Disclosure

Services are provided by a student practitioner for cosmetic and educational purposes only.

No medical claims are made.

Clients acknowledge and accept services at their discretion.
